Statement of Position/Position Summary:
This part-time Nurse Practitioner position is for you to provide primary care through house calls to our elderly population. They are often critically ill with multiple co-morbidities. These patients are unable to make it to a primary care provider's office or them doing so is a severe burden for them and/or their families - they "don't have to go to a family doctor; we go to them".
You will not only care for their acute needs, but manage any unmet medical needs and chronic conditions. A commitment to practicing preventative medicine is a must in order to maintain their best possible quality of life, and, to catch emergent conditions early to prevent those from becoming more serious.
Our collaborating physician(s) must be willing to engage in the related Agreements with you after their interview with you and their consideration.

Current Compensation (based upon current Medicare Reimbursements):
For patients in:
Community (non-facility) = $85 for existing patients per Medicare claim and $145 for new patients per claim.
Assisted Living facility = $90 for existing patients per Medicare claim and $145 for new patients per claim.
Skilled Nursing facility = $60 for existing patients per Medicare claim and $110 for new patients per claim.
Compensation paid per biweekly direct deposit once appropriate charting entered in EMR.
Medicare-mandated "Face-to-Face" visits related to hospice re-certifications are at $125 each.
Mileage in not paid by Mother's Touch, but may be claimed on your personal income tax return (confirm with your tax professional). Benefits are not available for part-time staff.

Qualifications/Requirements:
- Excellent interpersonal skills with the ability to "connect" with patients and their families.
- Self-motivated, dependable, and patient-focused.
- Advanced physical assessment, diagnostic skills, disease intervention, and medical management of chronically ill patients with multiple co-morbidities.
- Have a current, unencumbered, license through the Kansas Board of Nursing.
- At least two years geriatric, complex chronic care, hospitalist, or home-based primary care experience.
- A valid KS Driver’s License and daily access to a reliable and insured vehicle.
- Current DEA registration and be licensed to prescribe Schedule II medications.
- Meet Medicare/Medicaid credentialing requirements. You will be "assigning" your Medicare reimbursements to Mother's Touch in exchange for compensation directly from Mother's Touch.
- Pass a background check and drug screen.
- Be willing to accept Direct Deposit of compensations.
Duties:
- Conducts comprehensive health history and physical examination using advanced skills of assessment to determine diagnosis and appropriate plan of care for each patient.
- Order and perform laboratory and other diagnostic tests as appropriate to evaluate and diagnosis accurately while also considering physical signs and symptoms.
- Manage medical plan of care including prescribing medical devices, adjunct health/medical services, medications (including "controlled" medications), durable medical equipment, and other medical supplies as appropriate based on diagnosis and appropriate plan of care using evidence-based practice guidelines.
- Initiates and maintains accurate patient records in the electronic health record (EHR), including but not limited to past medical histories, progress notes, orders, procedures performed, and patient/caregiver education or instructions.
- Completes electronic progress notes within 48 hours of visit.
- Adopts a team-based approach to patient care through effective communication with office staff and care team members.
- Promotes continuity of care through collaboration with care team members and other providers, including specialist and community resources for optimal patient management.
- Educates patient and family/caregivers on health maintenance, disease processes, and self-management techniques using individualized teaching plans, patient coaching and motivation.
- Triage patient calls as needed via the telephone and electronic messaging during office hours.
- Participates in periodic evaluation of services rendered, including but not limited to, chart reviews, patient satisfaction, and clinical outcomes to identify areas for improvement.
- Applies knowledge gained to establish methods for improved quality of care.
- Recognizes, develops, and implements professional and community educational programs related to health care.
- Be willing and able to perform Medicare-mandated "Face-to-Face" assessments and recommendations for Mother's Touch Hospice as one of its staff members.
- May perform other duties as assigned or requested.
